Students must be enrolled in the School-Based Wellness Centers Program. The sports physicals will be held in the school’s health suite on Monday, 
Tuesday and Thursday mornings. Parents do not need to be present during the appointment. To schedule an appointment, please send an email to 
HowardSBWC@howardcountymd.gov  that includes:

• Student’s name
• Date of birth
• Parent’s or guardian’s name, phone number, and email address .

You will be emailed with the appointment date, the required HCPSS sports participation forms for the exam, and a SBWC Program enrollment form
