DL-300A (06-20)
The Updated MVA Learner's Permit application does not need a signature from the school. Just fill out, print, and parent/guardian must sign to verify school attendance. Application is a part of the Learner's Permit process within the MVA.

Naviance for Wilde Lake High School
Students need their complete Student ID Number and their password for the computers. Alternately, students can go to hcpss.me -> Go to Naviance App and log in

WLHS Transcript Process
Use this direction sheet to guide you through the process of requesting transcripts for colleges and universities.

TRANSCRIPT/STUDENT RECORDS REQUEST
Complete the Transcript Request form any time you need a transcript. You can list several requests/schools on one form. ❏ Completely fill out ​all columns​ of the Transcript Request form with the correct school information, deadlines, delivery type, and letter of recommendation. ❏ Be sure to provide a working phone number and email address where you can be reached if necessary. ❏ Email the Transcript Request form to our WLHS Registrar: Jackie Walker at ​jacqueline_frieson_walker@hcpss.org ❏ By submitting this form, colleges/schools will be placed in Naviance under “Colleges I’m Applying to” list. ❏ Please allow 20 school days to process transcripts requests.​ Requests made without 20 days to process may not be processed in time to meet your deadline. ❏ You may submit the Transcript Request form multiple times to add schools to your list. ❏ Midyear reports will automatically be sent to all schools listed on your Transcript Request form(s).

Waiver of Rights form
Complete the Waiver of Rights form and email to Student Services’ secretary, Sharon Wentland, at ​Sharon_Wentland@hcpss.org ❏ This form only needs to be completed ONCE. ❏ On this form, you must choose one of the following as it applies to: “​examine or obtain a copy of the letter(s), rating sheets and other evaluations​” ❏ “We do waive our rights” - you will ​not ​be able to read your letters of recommendation​ (Colleges/Universities prefer this answer) ❏ “We do not waive our rights” - you will be able to read your letters of recommendation
