The Howard County Health Department will be providing free sports physicals to Wilde Lake students wishing to play winter sports and who are enrolled in the School-Based Wellness Program.   If your student has Maryland Medical Assistance, only that program will be billed and there will be no charges or co-pays for families.   The sports physicals will be held in the school’s health suite on Monday, Wednesday and Thursday mornings.  To schedule an appointment, please send an email to HowardSBWC@howardcountymd.gov  that includes:
Student’s name
Date of birth
Parent’s or guardian’s name, phone number, and email address
Preferred date and time range for the appointment
You will be contacted with the appointment time and emailed the required HCPSS sports participation forms for the exam and a SBWC enrollment form if needed.  If you have questions, please contact Sharon Hobson, the program administrator, at 410-313-7238.
